Yes, there are 3 Tateeda entries in safe controls/

Yes, I'm site administrator for this site.

Sql server is on another box.

Please check your ShaarePoint web.config file.

There are should be Tateeda entries in safe controls.

Are you site administrator for this site?

Is SQL server on the same box as SharePoint?

Hi, after successful instalation , when i added webpart to page i recieved "unsafe webpart" error.

Reinstalled several times, checked assembly - have 3 Tateeda dlls.

Please Help me get it works.

1. you can modify it anyway you want! You got the source code

2. Build in Survey form desplay only one question per form by default

3. Default survey only available with MOSS.

There's a lot of things I don't like about SharePoint's built in surveys. What are some of the weaknesses in SharePoint Survey's that Dynamic Survey Forms improves upon?

Updated Release: Dynamic Survey forms for SharePoint. Alpha 1.0.1 (May 17, 2010)

Alpha release.
Before running installer create database from script attached in zip file.
In your web.config make sure your first connection string is pointing to databases where DB script was installed.
Default database name is TateedaForms.

If you see any errors installing for SharePoint Farm, just ignore it, web part should be there. Make sure you import web parts before adding theme to the page....

Detailed documentation is coming shortly.
